Reporting a workplace injury, incident or work-related death online is now easier.
Instead of choosing which report to complete before you begin, simply select the type of injury or incident you're reporting. Based on your selection, you'll be directed to the appropriate report.
What's new?
- One starting point: Report a work-related death, an injury requiring medical attention or an incident where medical attention is not being sought.
- Document uploads: Attach supporting documents or images directly to your report.
- Immediate claim number: Receive a claim number when you submit a First Report of Injury or First Report of Death.
- Downloadable copy: Save or print a copy of your completed report for your records.
You do not need an online account to report a workplace injury, incident or work-related death.
