WSI experienced a temporary system outage on Thursday, May 14, 2026, that impacted online services between approximately 7 a.m. and 11 a.m. CT.
To restore services, WSI rolled systems back to a previous point in time. This issue impacted multiple customer groups and may include:
- Online First Report of Injury submissions
- Incident Reporting
- Injured Employee submissions, including Income and Work Status (FL214) and Employment Contact Log
- Employer submissions, including Payroll Reports, Applications for Insurance, Verification of Nonemployment, Safety and Grant Applications, Designated Medical Provider updates, Premium Audits, and Account Updates
- Ergonomic Initiative Provider Documents
As a result, information submitted online during that timeframe may no longer be available in WSI systems.
Customers who submitted information during the affected timeframe should review their account activity or contact WSI for assistance before resubmitting requests.
Online services are now available.
