Prior Authorization Required for Long-Acting Opioid Medication

Effective February 1, 2015, WSI will require prior authorization of long-acting opioid medication.

Long-acting opioid medication requested within the first 90 days of the injury will require the prescriber to complete the Physician’s Request for Medication Prior Authorization (P2) form. The prescriber must fax the completed P2 form and supporting medical documentation to 701-328-3793.
