  • Claim number
  • Injured employee's SSN and date of injury
  • Injured employee's last name and first name, and date of injury
  • Injured employee's last name and date of birth, and date of injury
  
Enter the claim number with no dashes, for example, 2025123456.
  
Enter the nine-digit SSN with no dashes, for example, 123451234
  
  
 
Enter the date of birth in MM/DD/YY format, for example, 01/15/85
  
Enter the date of injury in MM/DD/YY format, for example, 01/15/85
      Results will reflect a +/- 31−day window from the date of injury.
