Discontinuation of Questionnaire Forms for Elective Lumbar Fusion Surgery

Effective November 1, 2017, WSI will discontinue the requirement for a provider to submit the Pain Questionnaire and Surgery Expectation Questionnaire forms as part of the prior authorization process for an elective lumbar fusion. WSI initially developed the questionnaires as adjuncts to the Lumbar Fusion Surgery Guidelines. The questionnaires were previously located under the Medical Treatment section of the WSI website, but will no longer be available.
