Application for Insurance (employers)

Use this online service to apply for workers' compensation insurance coverage electronically.

Claim Filing/First Report of Injury (workers, employers, and medical providers)

Use this online service to report a work injury to begin the claim filing process.

View your WSI workers' compensation account. This service provides an easy, confidential, and time-saving way for employers to view billing statements, pay premium and claim loss assessments online, access certificates of premium payment electronically, and review account information anytime, anywhere.

Search for employers insured with WSI. This service will identify whether an associated employer has an open policy with Workforce Safety & Insurance.

Fraud Reporting (workers, employers, and medical providers)

Reporting fraud benefits everyone. Our online fraud reporting service provides an easy and confidential way for customers to alert us to suspected fraudulent workers’ compensation activity.

Incident Report Image
Incident Report (employers)

An Employer should file an "Incident Report" if a worker reports an incident but does not seek immediate medical attention. WSI will pay the first $250 if the injury or incident is filed with WSI by midnight (Central Time) of the business day following the date of injury or incident. Should that worker seek medical attention and a claim is received within 14 calendar days of the incident, the employer would have the $250 assessment paid by WSI. Without the filing of the "Incident Report" the $250 medical assessment would still be charged if a claim is eventually filed. In addition, if the claim is filed more than 14 calendar days after employer notification, and the Incident Report wasn't filed, the medical assessment paid by the employer is $350 per claim.
