When a workplace injury occurs, a claim can be filed online.
To file a claim online complete the Claim Filing/First Report of Injury form.
This can be completed by the injured worker, employer, or medical provider.
The Claim Lookup service allows medical providers easy access to claim verification information.
Information retrieved is current as of the end of the prior business day.
