Claim Information
Injured workers need to immediately notify, (within 24 hours of the occurrence) their employer when an injury occurs. The claim also needs to be filed with WSI in one of three methods:

  • Online – complete the First Report of Injury and submit it online
  • By hand – obtain a First Report of Injury (FROI) form from the employer or WSI (also available as a fillable form on our website) Sign and date the form and return to WSI as soon as possible
  • By Telephone – call 1-800-777-5033 24 hours a day/weekends/holidays

If the claim is filed on-line, a claim number will be assigned immediately, otherwise the claim you will be notified in writing by WSI. The assigned claim number is only a confirmation of the claim being registered. All pertinent information relating to the injury will be reviewed and applied to North Dakota statutes for a decision of either acceptance or denial.

The injured worker, employer, or medical provider may complete the FROI and submit it to WSI. It is not necessary for all three section of the FROI to be completed on the same form. Each section can be completed by the appropriate entity on a separate form and submitted to WSI.
