File a Claim
To report a work-related injury online and begin the claim-filing process, complete the online First Report of Injury (OFROI) form. This is a quick, easy, and confidential way for workers and employers to submit claim information. Submitting online allows WSI to review the claim information faster, which helps speed up claim processing.
